What is Valclovir 500 Used For?

    • Valclovir 500 Tablet (Valacyclovir 500mg) is used for:
    • Recurrent Herpes Labialis (Cold Sores) – Episodic treatment to shorten outbreak duration
    • Genital Herpes (HSV-2) – Recurrent episode treatment and daily suppressive therapy
    • Suppression of Genital Herpes – 500mg once daily reduces recurrence frequency by up to 80%
    • Reduction of Herpes Transmission – Suppressive therapy reduces the risk of transmitting herpes to an uninfected partner
    • Chickenpox (Varicella) – In adults and adolescents
    • Note: For shingles (herpes zoster) or primary genital herpes, Valclovir 1000mg (higher dose) is typically required — see Valclovir 1000 Tablet

    How Does Valacyclovir Work?

    • Valacyclovir is a prodrug of Acyclovir — its superior oral bioavailability makes it the preferred oral antiviral for herpes infections:
    • After oral absorption, valacyclovir is rapidly converted to Acyclovir by hepatic and intestinal enzymes (valacyclovirase)
    • Acyclovir enters virus-infected cells and is preferentially phosphorylated by viral thymidine kinase
    • Acyclovir triphosphate inhibits viral DNA polymerase, blocking herpes virus DNA replication
    • Uninfected cells are largely unaffected — the specificity for viral thymidine kinase provides a favourable safety profile
    • Result: Viral replication stops, outbreak severity and duration are reduced, and viral shedding decreases
    • Valacyclovir achieves 3–5 times higher Acyclovir blood levels than equivalent oral Acyclovir doses, enabling fewer daily doses.

    How to Use Valclovir 500

Cold Sores (Herpes Labialis): 2g (4 × 500mg tablets) twice in one day, 12 hours apart — start at first sign of tingling

Recurrent Genital Herpes (Episodic): 500mg twice daily for 3 days — start within 24–48 hours of symptom onset

Suppressive Therapy (Genital Herpes): 500mg once daily continuously; review annually with your doctor

Administration: Oral tablet, with or without food

Important: Start treatment as early as possible when symptoms appear — antivirals are most effective in the first 24–48 hours

    Side Effects of Valclovir 500

Common Side Effects: Headache, nausea, abdominal pain (generally mild and transient)

    Less Common: Dizziness, vomiting, fatigue

Rare: Renal impairment (at high doses or with dehydration), thrombotic thrombocytopenic purpura/haemolytic uraemic syndrome (TTP/HUS) in immunocompromised patients

    Benefits

    • Higher bioavailability than oral Acyclovir (55% vs 10–20%) — fewer doses needed per day
    • Clinically proven to reduce cold sore healing time and genital herpes outbreak duration
    • Suppressive therapy (500mg daily) reduces transmission risk to uninfected partners
    • Well-tolerated with minimal side effects at standard doses
    • Fewer daily doses than Acyclovir tablets — twice daily or once daily for suppression vs five times daily for Acyclovir
    • Generic Valacyclovir provides identical efficacy to branded Valtrex at significantly lower cost
    • Active against HSV-1, HSV-2, and Varicella-Zoster Virus (VZV)

    Warnings and Precautions

    • Adequate hydration is important — maintain fluid intake to reduce risk of renal crystallisation, particularly at high doses
    • Renal function monitoring — dose adjustment required in patients with impaired kidney function
    • Immunocompromised patients — at higher risk of TTP/HUS; use under specialist supervision
    • Valacyclovir does not cure herpes — it suppresses viral activity; herpes remains latent and outbreaks can recur after stopping
    • Herpes transmission can still occur even with suppressive therapy and during asymptomatic viral shedding
    • Not for newborns — neonatal herpes requires IV Acyclovir under specialist management
    • Driving/machinery — dizziness is a rare side effect; assess individual response

    Drug Interactions

    • Probenecid and Cimetidine – Reduce renal clearance of Acyclovir; may increase plasma Acyclovir levels; monitor for side effects
    • Nephrotoxic drugs (NSAIDs, aminoglycosides, ciclosporin) – Increased renal toxicity risk; avoid concurrent use or monitor kidney function closely
    • Mycophenolate mofetil – Combined use in transplant patients may increase both drugs' concentrations; specialist management required
    • No significant interaction with most common medications at standard doses

    Storage Information

    • Store at room temperature 15°C–25°C
    • Keep away from moisture and direct sunlight
    • Keep out of reach of children
    • Store in original blister packaging
    • Do not use after the expiry date printed on the packaging

    Frequently Asked Questions

Q1: What is Valclovir 500 (Valacyclovir 500mg) used for?

Valclovir 500 is used to treat herpes simplex virus infections. Specific uses include: episodic treatment of cold sores (HSV-1 labialis) as a high-dose one-day regimen; episodic treatment of recurrent genital herpes (HSV-2) for 3 days; daily suppressive therapy to reduce genital herpes recurrence frequency; reducing herpes transmission to uninfected partners; and treating chickenpox in adults.

Q2: How does Valacyclovir differ from Acyclovir?

Valacyclovir is the L-valine ester prodrug of Acyclovir. When taken orally, it is rapidly converted to Acyclovir in the body. The key difference is bioavailability: oral Acyclovir has only 10–20% bioavailability, meaning most of the dose is not absorbed. Oral Valacyclovir achieves 55% bioavailability and provides 3–5 times higher Acyclovir blood levels. This allows Valacyclovir to be dosed twice or once daily, compared to five times daily for Acyclovir — a major practical advantage.

Q3: How quickly does Valclovir 500 work on a cold sore?

When started at the first sign of a cold sore (tingling or itching before any blister appears), the high-dose one-day regimen (2g twice, 12 hours apart) reduces healing time by approximately 1 day compared to no treatment. Valacyclovir works best when started immediately — delay reduces its benefit. If blisters have already formed, it still reduces the duration and severity of the outbreak but the effect is less pronounced.

Q4: What is the correct dose of Valclovir 500 for genital herpes suppression?

For daily suppressive therapy in adults with normal kidney function, the standard dose is 500mg once daily, taken continuously. This reduces the frequency of genital herpes recurrences by 70–80% in most patients and reduces viral shedding (and therefore transmission risk). Annual review is recommended — some patients may eventually be able to stop suppressive therapy as recurrence frequency naturally decreases over time.

Q5: Does Valclovir 500 work for shingles (herpes zoster)?

Valclovir 500mg (500mg dose) is not the appropriate dose for shingles. Shingles requires 1000mg (1g) three times daily for 7 days — this is provided by Valclovir 1000 Tablet. The higher dose is needed to achieve antiviral concentrations sufficient against the varicella-zoster virus in herpes zoster. See Valclovir 1000 Tablet for shingles treatment.

Q6: What is the difference between Valclovir 500mg and Valclovir 1000mg?

Valclovir 500 and Valclovir 1000 both contain Valacyclovir but at different strengths for different indications. Valclovir 500mg is used for cold sore episodic treatment (high one-day dose of 4 tablets), recurrent genital herpes (500mg twice daily for 3 days), and genital herpes suppression (500mg once daily). Valclovir 1000mg is used for shingles (1000mg three times daily for 7 days) and primary or severe genital herpes episodes (1000mg twice daily for 10 days). The higher strength reduces the number of tablets per dose.

    Q7: Can Valacyclovir cure herpes?

No. Valacyclovir cannot cure herpes. Herpes simplex virus (HSV-1 and HSV-2) and varicella-zoster virus establish lifelong latent infection in nerve ganglia. Valacyclovir suppresses viral replication during treatment, reducing outbreak severity and frequency, but the virus remains in the body. Outbreaks may resume after stopping treatment, particularly if triggered by stress, illness, or immune suppression.

Q8: Is it safe to take Valclovir 500 every day for suppression?

Yes. Daily suppressive therapy with Valacyclovir 500mg has been studied in long-term clinical trials (up to 3 years) and shown to be safe and well tolerated. It is an established and widely recommended approach for people with frequent herpes recurrences (more than 6 per year) and for reducing transmission risk to partners. Annual reassessment with your doctor is recommended to determine whether continued suppression is needed.

Q9: Does Valacyclovir prevent herpes transmission to a partner?

Daily suppressive Valacyclovir significantly reduces — but does not eliminate — the risk of transmitting herpes to an uninfected partner. In a landmark clinical trial, 500mg daily reduced transmission of symptomatic genital herpes to susceptible partners by 75%, and overall herpes acquisition (including asymptomatic) by 48%. Suppression combined with condom use provides the most comprehensive risk reduction.

Q10: What should I do if I miss a dose of Valclovir 500 suppressive therapy?

If you miss a dose of suppressive Valacyclovir, take it as soon as you remember on the same day. If you do not remember until the next day, skip the missed dose and continue with your regular schedule. Do not take two doses in one day. Missing occasional doses during long-term suppressive therapy is unlikely to cause an immediate outbreak but reduces the overall protective effect over time.

Q11: How should Valclovir 500 be stored?

Store at room temperature between 15°C and 25°C. Protect from moisture and direct sunlight. Keep out of reach of children. Store in the original blister packaging. Do not use after the expiry date printed on the pack.

Q12: Do I need a prescription for Valclovir 500 (Valacyclovir)?

Yes. Valacyclovir is a prescription medicine in most countries. A healthcare provider should confirm the diagnosis (herpes simplex or herpes zoster) and prescribe the appropriate dose and regimen. Kidney function should be checked before prescribing in patients with renal impairment, as dose adjustment may be required.
