What is Instgra (Dolutegravir 50mg) Used For?
- INSTI (integrase strand transfer inhibitor) β preferred third agent in WHO first-line HIV regimens
- HIV-1 treatment in adults and adolescents β combined with a dual NRTI backbone (3TC/TDF, FTC/TAF, 3TC/ABC)
- HIV-1 treatment in virologically suppressed adults as part of simplified dual therapy with Lamivudine (DTG+3TC)
- Safe and recommended throughout pregnancy by WHO and most national guidelines
How Does Instgra Work?
- Binds the active site of HIV integrase enzyme (strand transfer step), preventing insertion of viral cDNA into the host cell chromosome
- Highest genetic barrier to resistance of all INSTIs β requires multiple rare integrase mutations for resistance to develop
- Does not require pharmacokinetic boosting (no ritonavir or cobicistat needed)
- Rapid viral load decline β 80%+ of patients achieve undetectable viral load within 24 weeks
How to Use Instgra
Dosage: 50mg once daily; increases to 50mg twice daily when taken with rifampicin or efavirenz
Timing: Ideally on an empty stomach; antacids, iron supplements, calcium, and magnesium must be separated by at least 2 hours (reduce Dolutegravir absorption)
Precaution: Metformin levels increase significantly when started with Dolutegravir β monitor blood glucose and adjust metformin dose accordingly
Side Effects of Instgra
Common Side Effects: Insomnia, Headache, Nausea, Diarrhea, Dizziness
Serious Side Effects (seek medical help): Severe neuropsychiatric symptoms (suicidal thoughts, psychosis), Serious hypersensitivity reaction with hepatotoxicity, Immune reconstitution inflammatory syndrome
Benefits
- Highest resistance barrier of any INSTI β clinical resistance in treatment-naive patients is exceedingly rare
- No pharmacokinetic booster required β fewer drug interactions than boosted PIs
- Safe in pregnancy β WHO-recommended throughout all trimesters
- Compatible with simplified dual therapy (DTG+3TC β GEMINI trial data)
- Proven rapid and durable viral suppression across diverse patient populations
Warnings and Precautions
- Antacids/iron/calcium/magnesium: reduce Dolutegravir absorption β must be separated by β₯2 hours
- Rifampicin and Efavirenz: reduce Dolutegravir levels β increase dose to 50mg twice daily
- Metformin: Dolutegravir inhibits OCT2 transporter, increasing metformin concentrations β reduce metformin dose or monitor closely
- Neuropsychiatric effects: insomnia and headache common; significant depression or suicidal ideation requires medical evaluation
- Serum creatinine: modest increases (not true kidney damage β Dolutegravir inhibits tubular creatinine secretion)
- Hypersensitivity with hepatitis: rare but serious β stop immediately if rash with systemic symptoms
Drug Interactions
- Rifampicin: requires Dolutegravir 50mg twice daily dosing
- Efavirenz (and other NNRTIs that induce CYP3A4/UGT1A1): requires 50mg twice daily
- Antacids (Al/Mg), iron, calcium, multivitamins: reduce absorption β separate by β₯2 hours
- Metformin: increased levels β monitor renal function and reduce metformin dose
- Related HIV regimens at Medionix Pharma: Twinaqt (DTG+3TC), Xapavir-LT (DTG/3TC/TDF), Spegra (DTG/FTC/TAF), Acriptega
Storage Information
- Store at room temperature (below 30Β°C), away from direct light and moisture
- Keep in original packaging out of reach of children
- Do not use after the expiry date
Frequently Asked Questions
Q1: What is Instgra Dolutegravir 50mg used for?
Instgra is a branded formulation of Dolutegravir 50mg, an INSTI used as a key component of HIV combination therapy. It is prescribed as part of complete triple therapy regimens and is a WHO-recommended first-line agent.
Q2: How does Instgra/Dolutegravir differ from older HIV drugs?
Dolutegravir works at the integrase step β a different HIV replication target than older NNRTIs and PIs. It requires no pharmacokinetic boosting, has the highest genetic barrier to resistance among INSTIs, fewer drug interactions than PIs, and is safe during pregnancy.
Q3: How is Instgra 50mg taken?
Take 50mg once daily, ideally on an empty stomach, at a consistent time each day. In patients on rifampicin or efavirenz, the dose increases to 50mg twice daily. Antacids, iron, calcium, and magnesium supplements must be separated by at least 2 hours.
Q4: What resistance properties make Dolutegravir durable?
Dolutegravir requires multiple integrase resistance mutations to lose activity β a very high genetic barrier. Clinical resistance to Dolutegravir in treatment-naive patients is exceedingly rare, making it one of the most durable HIV drugs available.
Q5: What are the most common side effects of Instgra?
Insomnia and headache are most commonly reported. Nausea, diarrhea, and dizziness can occur. Neuropsychiatric effects (anxiety, depression, mood changes, vivid dreams) affect a minority of patients, particularly those with pre-existing psychiatric conditions.
Q6: What neuropsychiatric effects require medical attention?
While insomnia and mild mood changes are common, significant depression, suicidal thoughts, or psychosis warrant immediate medical evaluation. Some patients require switching to an alternative INSTI if neuropsychiatric effects are intolerable.
Q7: What monitoring is needed during Instgra therapy?
CD4 count and viral load at baseline, then 4β8 weeks, then every 3β6 months. Serum creatinine may increase slightly (Dolutegravir inhibits tubular secretion of creatinine β this is not a sign of kidney damage). Liver function at baseline and periodically.
Q8: Does Dolutegravir interact with HIV medications?
Dolutegravir has fewer interactions than PIs. Key interactions: rifampicin and efavirenz require 50mg twice daily dosing. Antacids, iron, and mineral supplements reduce absorption. Metformin levels increase significantly β monitor and reduce metformin dose.
Q9: Is Instgra safe during pregnancy?
Yes. After initial concerns about neural tube defects were refuted by larger studies, Dolutegravir is now recommended in pregnancy by WHO and many national guidelines. Discuss with your HIV and obstetric specialists.
Q10: Can Instgra be used alone as monotherapy?
Dolutegravir dual therapy (with Lamivudine only) is an evidence-based option for virologically suppressed patients in some guidelines. However, standard triple therapy remains the default β monotherapy with Dolutegravir alone is not recommended.
Q11: How should Instgra 50mg be stored?
Store at room temperature away from light and moisture. Keep in original packaging out of reach of children.
Q12: Do I need a prescription for Instgra?
Yes. Dolutegravir requires specialist HIV prescribing, baseline viral load and CD4 count assessment, and regular monitoring throughout treatment.
